It looks like a charm/pennant with a thin, silver or white gold chain wrapped around her ankle. Cafepress has a bunch of jewelry you might check out. There's another site that has real, upscale, gold & silver cuckold jewelry, but I couldn't find the link right off hand. When I do I'll add it to this one.
Also check pinterest.com as they deal with upscale jewelry like this.

Yeah, see what you mean. Charm bracelets are ok with all the added charms, but when it comes to anklets, keeping it simple (1-2 charms) and a higher quality i.e. real gold, instead of gold plated, seems to look better, especially on nice ankles like she has. This is one of those "you get what you pay for" type products ... personally, my lady only gets the best. Good luck!
