Biggest concern with a chastity cage that's too small is he might be able to slip out from the back. A urethral tube or a Prince Albert piercing can prevent this.

For hygiene, I recommend he soaks in baths every few days (ideally daily) and probably a weekly hygiene unlocking inspection.

I have tried many chastity cages and unless it is a piercing or some sort of anti pullout ring I feel it is a joke and you can escape from it .
