Yes it hurts…..I have years of experience, have paid high $$$ for custom made/measured cages and it doesn’t matter….I still get woke up in the middle of the night several times a night with pain from an erection in my cage at times (not every night). Don’t know if that’s him but that’s not a small penis. Nor is it flaccid…actually looks substantially erect there, and that’s definitely not the time to put on a cage, gauge fit,.., espesh to properly evaluate the fit of a base ring. His erection is pushing the ring outward there, and arousal is inevitable…some of that will eventually happen no matter what. But from what I see, he can probably go with a somewhat smaller ring, and the thing to do is reinforce that ring with a strap so that it stays closer and snug to the pelvis/pubic area.
