If your cage allowes you to get an errection its possible that it hurts. If your cage is optimal sized, there is no space for this.

But even with optimal size, dick is full inside cage. If you get erected there's no space for that. So what happens in that cases? If there is no space your dick coul get hurt because it stays so tight skimming constantly with cage?

That’s not necessarily so. A man’s build, the usual hang of his scrotum/balls, the fit of the ring, how snug and how close to the pelvic base it sits, all these things figure. You can definitely become erect and very uncomfortable in certain cages that are a great fit most of the time.
