That would be so difficult, awkward, and staged that one one would have a good time. For me, it is easy to make DP happen in the natural heat of the moment. I've never been "airtight", but adding one more guy by my head would be easy enough, even if it didn't seem like part of the natural flow of a sexual encounter. But adding a fourth guy would require a director, someone flicking that Twister spinner ("okay, everyone: right hand red!"), and someone from the Guinness Book to make sure everyone actually stayed balanced for x many seconds. In the seventies, there was this weird thing about seeing how many people could fit inside of a phone booth. (Where does this guy stand? Where can my hand go? Ok everyone, take turns thrusting round-robin!) In my sex life, I don't want something so planned that I need mathematicians and engineers planning it. I want to get lost in the moment.
Porn directors may understand that they can make some money off of a shoot that offers that to their viewers. But I would bet that basically no woman ever includes that as a part of the flow of a normal sexual encounter. Just my opinion!
