I don't think it is a disease but I do think a wide range of factors can lead to a cuckoldling lifestyle all ranging from both positive and negative variables.
Some people are born with alpha personalities and some more submissive. The lifestyle then becomes slightly more appealing to submissive types who would rather yield power to a more dominant *******.

On the more sinister side some cuckolding can stem from traumatic experiences in childhood and adolescences. Bulling and abuse can make an individual feel less than those effects manifest themselves into a sexual lifestyle that reflects how the individual was made to feel back in their youth. In those cases I would say therapy might actually be a needed to flesh out the more deep rooted issue.
So I think cuckolds find themselves in the lifestyle through various routes and it is up to the cuckold to understand where his desire comes from.
